Anthem security breach: what you need to know CSEA has been made aware that Anthem, the parent company of Empire Blue Cross/Blue Shield, has experienced a data breach that has potentially impacted ALL Anthem (formerly Wellpoint) customers and members, including the following: • NYSHIP Empire Plan enrollees; • “Blue Card” (Blue Shield insurance products used in a state where Anthem, Inc. operates) Information accessed may have included the following: • names • dates of birth • Social Security numbers • health care ID numbers • home addresses • email addresses • employment information, including income data Anthem is in the process of identifying the number of affected members and is working with the FBI and an outside data consulting investigation firm. Since Anthem is still in the early stages of its investigation regarding this external cyber attack, the following website is the most accurate source for more information: http://www.anthemfacts.com. Accessing identity theft and credit monitoring services Anthem is working with AllClear ID, a leading and trusted identity protection provider, to protect your identity for two years at no cost to you. AllClear ID will provide identity theft repair and credit monitoring services to current or former members of an affected Anthem plan dating back to 2004. April 30 is deadline for 2014 Empire Plan claims Empire Plan enrollees have until April 30, 2015 (120 days after the end of the calendar year) to submit medical expenses that were incurred during the 2014 plan year to one of the following: United HealthCare Service Corp. P.O. Box 1600 Kingston, N.Y. 12402-1600 For the Empire Plan Basic Medical Program, the Home Care Advocacy Program (HCAP) and for non-network physical therapy or chiropractic services.
