Help us help you! Keep your EBF records up-to-date It is important that your CSEA Employee Benefit Fund (EBF) records for your dental and vision benefits are up-to-date. Maintaining the accuracy of your EBF member and dependent records is the responsibility of the plan member. A quick phone call or the submission of supporting enrollment documentation to the EBF can often prevent eligibility and claim errors. Have you moved or experienced another major life event? Many issues arise when there is inaccurate dependent information on file. It is very important that you notify the EBF with any changes to your family such as marriage, the birth of a child, or moving to a new address. It is also necessary to notify the EBF of a legal separation or divorce. The EBF does not share individually identifiable health information with your employer. Failing to notify the EBF of changes can create issues with your benefits. Members must notify the EBF of address changes. Example: You moved and your address has changed. The EBF issues and mails Explanation of Benefits (EOBs) as well as other important information regarding your benefits to the address on file. EOBs are a written statement used to inform the member and/or dependents after services are rendered as to what provider charges were submitted to the EBF and what has been paid by the EBF. Members must remove a spouse from plans when divorced. Example: An ineligible former spouse uses your EBF benefits. If you have divorced and failed to notify the EBF of this change in marital status and the ineligible former spouse uses your benefits, the plan member would be responsible to pay any benefits that have been paid out after the date the court has legally declared that the divorce proceedings are final. How do I notify the EBF of a change in dependent status? Contact the EBF Member Services Department at 800- 323-2732 and our customer service staff will be able to assist you. In some cases, you will need to fill out and send different forms to make changes. All forms and other important plan information can be found on EBF’s website at www.cseaebf.com.
