EBF dental coverage: some tips to keep you smiling CSEA Employee Benefit Fund (EBF) dental coverage does not require the use of a participating dentist but you may be able to enjoy significant cost savings by using one. Participating dental providers must agree to accept EBF plans as full payment for all covered services. Non-participating dentists may bill the difference between the office’s normal charges and the EBF fee schedule allowance if there is one. Payment amounts are based upon the fee schedule for your dental plan. The fee schedule can be viewed at EBF’s website at cseaebf.com. Reading your plan documents allows you to be an informed consumer so you can make important decisions about your dental care. It also provides you with the information you need to work with your provider. If you haven’t received a copy of your EBF plan, download a copy and review it carefully. If you are unable to access our website, call EBF at 800-323-2732. Using coverage out of state Are you using your EBF dental coverage out of state? Here’s how to find a participating provider. Using the “Provider Search” tool from the home page of cseaebf. com, select the “State” option, then click “Find Providers,” and now you will be able to view EBF participating provider network outside New York state. Only states with providers are listed. In addition to searching by state, you can search by choosing a municipality or county. Completed dental claims must be submitted on a universal dental claim form or electronically. Paper claims must be fully completed and the provider must fill in their portions of the form to be processed. Paper claims can be mailed to CSEA Employee Benefit Fund, P.O. Box 489, Latham, NY 12110-0489. All providers are able to contact the EBF at 800-323- 2732 and speak with our dental customer service department if they have any questions or for more information on how-to file electronic claims.
